The Home Infusion Clinical Liaison position is responsible for creating and maintaining positive referral relationships in order to increase Home Infusion Referrals within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and other individuals to assess appropriateness home infusion, facilitate patient care transitions onto service and ensure there is appropriate documentation for ordered infusion therapy.
