The Triage RN reports to Associate Clinical Director. He/she is responsible for triaging patient and caregiver telephone calls, assisting in the solution, palliation of symptoms and provision of education. Assists with identifying patient/family care needs and for providing nursing and supportive care for the terminally ill patient and their families, in conjunction with case managers and physicians. Maintains relationships with families and patients.
