Job Description Summary The Social Worker is primarily responsible for assessing the psycho/social needs of patients/families/caregivers, educating patients, families, and Millennium employees in understanding what type of community resources are available and coordinating with the appropriate resources to assist the patient. Resources include but are not limited to food, shelter, utilities, pharmaceutical financial needs.
