Liquid Personnel is recruiting an Adult Social Worker to join its client’s Hospital team, based in Manchester. What will your responsibilities be? Managing hospital discharges - completing timely Care Act assessments, coordinating safe discharge plans and ensuring appropriate community support is in place.
