The Transplant Coordinator III-NP, under the direction of a collaborating physician, assists in the care of patients with a variety of end stage organ diseases. Coordinates and participates in the medical/surgical management in either inpatient and/or outpatient setting of transplant and/or end stage organ diseases. Performs comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's responses.
