Job Description: Program Summary Beth Israel Lahey Health Primary Care’s Extended Care Community Program (ECCP) provides patients with personalized, high quality medical care within rehabilitation and long-term facilities. We provide patients with the essential medical care during their stay and support them in transitioning back to their primary care physician upon discharge home or to another appropriate care setting.
