DISTINGUISHING FEATURES OF THE CLASS: This position exists in the County Certified Home Health Agency and involves responsibility for the delivery of varied social work services to patients and families. The duties involve the responsibility for providing initial psychosocial evaluations, on-going psychosocial support, direct casework services, community education, outreach and referral. The work is performed under the general supervision of the Director of Patient Services.
