About this opportunity

The Palliative Care Community Health Worker (CHW) is a member of the core interdisciplinary team whose primary function is to assist in providing direct community health support services and conduct goals of care conversations with the patient and family within the palliative plan of care. The CHW provides social support and informal counseling, advocates for individuals and community health needs of patients with serious illness.
